Western Hemisphere Travel Initiative: New Requirements for Travelers

Emergency Passport Issuance

If you discover that your passport has been lost or stolen, please contact the Interests Section's American Citizen Services Unit (833-3552; or 5280-5791/833-2302, if after working hours) as quickly as possible that same day or (if after hours) the next working day. Bear in mind that replacement passports are issued after working hours only in extreme cases of emergency and applicants will be charged an additional fee of $235 per hour for time spent in issuing a passport after normal working hours. Requirements for issuance:

  • 2 photos 2" by 2"
  • Evidence of identity (government issued photo ID, driver's license, etc.)
  • Evidence of citizenship (birth/naturalization certificate, passport copy)
  • Completed form DS-11 (Application for Passport)
  • Completed form DS-64 (Statement Regarding Lost or Stolen Passport)
  • $97 in U.S. currency or equivalent in Cuban Convertible Pesos (CUC) (cash only; checks or credit cards are not accepted)

Photos can be obtained at the Edificio FOCSA at 17th and M, and also at 23 and O; or at the Galerias Paseo in front of the Melia Cohiba Hotel in Vedado. The cost is approximately $3 for 6 photos.

First Time Application

If you are applying for a U.S. passport for the first time (in connection with a claim to citizenship), you should contact our American Citizen Services Unit at 833-3552 to schedule an appointment for an interview with a consular officer. A consular assistant will explain to you what documentation you should bring with you to the interview.
